MISSIONS :
- Provide information on home services to potential beneficiaries or redirect demand
- Analysing needs in terms of home help and care, in consultation with the beneficiary and their family, and helping to put these in place
- Organising and leading coordination meetings
- Creation and maintenance of the computerised coordination file
- Work with the network present throughout the country
- The coordination function reports to the A.S.D. Management.
PROFILE:
- Hold a diploma at least non-university higher education, with a paramedical or social orientation social workers, social nurses, occupational therapists, etc.
- Driving licence and private vehicle essential
- Live close to or in Walloon Brabant because of frequent travel
- To be in APE conditions
- Demonstrate a capacity for openness and collaboration.
- Ability to work in a team with professionals from different disciplines
- Ability to organise and run meetings
- Be able to deal with emergency situations
- Demonstrate good oral and written communication skills
- Be able to assume a functional, non-hierarchical role in relation to departments and/or service providers
- Be able to take the initiative and work independently
- Respecting the functions and tasks of those involved in coordination
- Demonstrate administrative and organisational skills in following up files.
- Be able to use the FASD software (after training) and demonstrate good computer skills in this area.
