Upon receipt of a request, a member of the Coordination Centre will contact the beneficiary or their carer to arrange a meeting and assess their needs.

In collaboration with the beneficiary, their family and friends, and their attending physician, a support and care plan is proposed.

The centre works with all psycho-medical-social stakeholders, without exclusivity or obligation. Each beneficiary is therefore free to choose their service providers.
