Organisation designed for wellbeing at home

The Home-Based Geriatric Support Service (SAGàDom) prioritises your well-being and independence. It is aimed at people aged 60 and over who are experiencing a loss of independence, a complex situation at home or a difficult event.

Each beneficiary is supported by a multidisciplinary team of experienced professionals:

Case manager nurse: coordinates your care pathway, in conjunction with your support and care team. Therapeutic education, counselling, direct care support.
Psychologists: They support you and your loved ones, helping you to maintain your bearings in your living environment and get through difficult times. Self-esteem, acceptance, support during bereavement and loss of independence.
Occupational therapists: They ensure your comfort and mobility in complete safety, in order to maintain or restore your independence in daily activities. Autonomy, mobility, comfort.

Depending on your needs, these professionals can work separately or as a team, ensuring an interdisciplinary approach and consistent follow-up.

SAGàDom is part of the ASD home care network in the Verviers district, ensuring smooth coordination with other services.

A telephone hotline is available 24 hours a day, 7 days a week at 087 32 90 90, in order to ensure continuity of care and respond to emergencies or specific needs.
