Upon receiving a request, one of the service members contacts the user or their caregiver to arrange a meeting and assess their needs.

In collaboration with the user, their family and friends, and their attending physician, a support and care plan is proposed.

The service works with all psycho-medical-social actors, without exclusivity or obligation. Each user is therefore free to choose their service providers.

Thanks to a comprehensive overview of the situation and knowledge of the psycho-medical-social sector, our team ensures the implementation of a personalised, multidisciplinary and evolving care and support plan.
